Imprimer deux copies du formulaire avant de l'envoyer de façon électronique, une copie pour vos dossiers et une à inclure avec votre paiement. Il est aussi possible d'envoyer ce formulaire en pièce jointe (copier et coller en MSWord) ou par télécopieur ou par la poste. L'inscription est sujette aux conditions ci-dessous.
Print two copies of the completed form before you submit it to SPEAQ online, one for your records and one to send with your payment - alternately you may email (copy and paste into MSWord), fax or mail the completed form. Registration is subject to the conditions listed below.

Conditions:

* Les frais d’inscription au congrès comprennent la cotisation annuelle (membres réguliers 50 $, membres retraités 25 $) et les taxes.
* La cotisation est valable jusqu’au début du congrès 2009.
* Les personnes à la retraite doivent fournir une preuve de leur statut.
* Aucun chèque postdaté ne sera accepté.
* Le reçu officiel sera remis sur place avec la trousse d’inscription au congrès.
* Toute demande de remboursement devra être faite par écrit dans les 14 jours suivant le congrès.
* Des frais d’administration de 20 $ ainsi que la cotisation ne sont pas remboursables.

* Convention costs include annual membership dues (regular member $50, retired member $25) and taxes.
* Membership expires immediately before the 2009 Convention.
* Retirees must provide proof of their status.
* Postdated cheques are not accepted.
* An official receipt will be included in the convention package.
* All refunds must be requested in writing within 14 days of the convention.
* A $20 administration fee and membership dues are not refundable.
